Friends of the Foundation
The Community Health Foundation is beginning a brand new annual fundraising campaign called Friends of the Foundation. In order to control costs and best steward our resources, the Foundation will only have our fundraising event, Meal to Heal, every other year. The Foundation, however, knows there is incredible need in Kay County and invites you to become a Friend of the Foundation to continue to support our health-improving efforts.
There are eight levels of “Friends” in our new giving program with many benefits offered to you and/or your business at each level. We hope that you will give careful consideration to your tax deductible contribution this year!

Two ways to become a Friend of the Foundation:
1. PRINT & MAIL: Click the pdf button below to download and print out our 2024 Friend of the Foundation form. Complete the form and drop in the mail with your financial contribution. Mailing address: PO Box 828, Ponca City, OK 74602.
2. ONLINE: Click the Sign Up Now button below to select your Friends of the Foundation level and securely donate online.
A Foundation donor display can be found in the lobby of the Alliance Health Hospital in Ponca City. The Foundation recognizes lifetime donors of $1000 or more in this display. We truly appreciate the many community members who have contributed to the Foundation and improving the health of Kay County.
