The Community Health Foundation invests in the current and future healthcare professionals in our community to help improve health in Kay County. Healthcare professionals who are currently working in Kay County who desire to attend educational courses that will aid in their professional practice, and who DO NOT have their continuing education courses paid for by their employer, may apply for reimbursement for continuing education. A maximum of $850 per provider per year may be reimbursed. Application submission does not guarantee reimbursement and any reimbursement is solely up to the Board of the Community Health Foundation of Kay County. Healthcare professionals may apply for funds prior to taking the class and can receive an approval of reimbursement, but reimbursement will not be provided until a receipt for course enrollment and a certificate of completion are provided. This documentation must be provided within 30 days of course completion in order for reimbursement to be issued.


Eligibility
All criteria below must be met to apply. Applications submitted for reimbursement do not guarantee reimbursement. Any reimbursement must be approved by the Community Health Foundation Board and will be based on potential community benefit of the course and funding availability.
1. Applicant must work in Kay County, Oklahoma in a healthcare profession.
2. Applicant must complete the Reimbursement Application in its entirety.
3. Applicant must explain how the continuing education will impact his or her healthcare practice.
4. Applicant must provide a copy of the course description or brochure with the application.
5. Applicant must agree to provide both a receipt of class registration and a certificate of course completion within 30 days following the training.
