Each year, the Community Health Foundation accepts donation requests from local organizations and agencies.

The Foundation Board carefully reviews each request, considering its alignment with the Foundation’s mission and values, its potential impact on the community, and the applicant organization’s financial responsibility.

CHF Annual Grant Application

The Community Health Foundation (CHF) awards grants to organizations whose programs improve the health of Kay County residents and align with one or more of our strategic priorities: Physical Activity, Nutrition, Mental Health, and Access to Care. This larger grant request is only once per year.

Eligibility

Serve residents of Kay County.
Be a 501(c)(3) nonprofit, governmental entity, school, other tax-exempt organization, or business
Be in good financial standing.
Complete all sections of the application.

Applications are due annually by August 15. Funding requests are reviewed by the Community Health Foundation Board of Directors, and applicants will be notified by the end of September, or early October.

Applicants may be asked to meet with the Executive Director to discuss their proposal. Organizations awarded funding must submit an Outcome Report by July 31 following the award year.

Additional Funding Opportuntities

Additional funding may be available throughout the year, depending on our budget and grant requests received. Funding is awarded on a first-come, first-served basis, and all requests must align with our priorities: Physical Activity, Nutrition, Mental Health, or Access to Care. Must meet all the requirements above.

Community Health Education Fund

The Community Health Foundation awards funding to local organizations who provide community health education that supports the Foundation's mission of improving health in Kay County, Oklahoma.

Applications for funding for community education are accepted on a rolling basis. Applying organizations must be in good financial standing and complete all parts of the application to be considered.

Providing compassionate end-of-life care and grief support at no cost to patients and families in our community.

Helping remove financial barriers to essential medications, ensuring community members can access the prescriptions they need to manage their health and well-being.

Strengthening community families by supporting the Parents as Teachers Curriculum.

Supporting first responders education with advanced emergency response equipment and kits.

Fighting food insecurity by providing an support to our emergency food bank.

Providing meals to children who may not have reliable access to food on weekends and school holidays.

Kaw Nation Library

Promoting holistic wellness with yoga, meditation, and stress-reduction workshops.
